No client information

Please do not submit client names, clinical information, diagnoses, records, or other protected health information through this site. These forms are not a secure clinical system and are never an appropriate place for client data.

Photo and video consent

This gathering will be photographed and filmed in a public setting. By checking the photo and video consent box, you agree that your image, likeness, and voice may appear in photographs and video created at the event and may be used for mental-health awareness, event recap, press, and promotional purposes by the organizers and their partners, without compensation. You may contact the organizers at any time with questions or requests about your images.

Secure submission and organizer-only access

Forms submit over an encrypted connection. Registration, volunteer, sponsor, and referral records are protected by database access rules so that only signed-in, approved organizers can view them. Uploaded sponsor logos are stored privately and viewed only by organizers.

Spam protection and required consent

Our forms include hidden spam checks and timing checks to reduce automated submissions. Consent checkboxes are required where indicated so we have a clear record of your permission before we contact you or use event media.
